The FCA has created a new form on its website for use by firms and insolvency practitioners making a Principle 11 notification. The purpose of the form is to get the information the FCA will need to assess how to deal with the notification up front. The questions asked include questions to determine whether there has been a breach, and give a drop down menu for the reporter to select one of several categories of potential issue.
