The latest edition of Market Watch looks at completeness and accuracy of instrument reference data. It looks at the most common rejection messages and the number of rejections, and suggests best practice for submitting firms on submitting accurate issuer LEIs and proper classification of instruments. The FCA also says it is concerned it has not been receiving as many breach reports as it might have expected.
